Guardian Consent Agreement

From the following options, please choose the program that you wish to register. Please check in the fee box for the plan of your preference. There is a one-time $30 registration fee for children who are new to our studio. Please note: The fees included are based on an automatic payment plan from a checking or credit card account. If you choose to be invoiced and pay by cash or check, you will be charged a monthly invoice fee of $20.

Enrollment is accepted anytime during the year.Monthly tuition is based on a 48 week year and has already been prorated to reflect the 4 weeks a year we are closed for the holidays. All prices are subject to change.

POLICY: How do I pay?

We accept bank cheques and credit card payments (VISA,MASTERCARD,AMEX) in person at our location. If you wish to mail your payment please send to our address below.

Initials PAYMENT PLAN

Pay automatically through your credit card. Monthly fees must be paid in full on the 1st of each month. $25 service fee will be applied to each returned item (declined credit cards, invalid check)

Initials ABSENCES AND CANCELLATIONS

You are allowed to freeze your account for 1 months without having to pay the registration fee repeatedly when you return. We do not credit for time missed other than when it was notified in advance due to emergency reasons. If your child is ill, please call us before 1:00pm. Please call each day of absence.Please make sure all absences are notified to the front desk directly, not handled by site instructors.

15 working days notice in writing is required for all cancellations to the membership. Without proper written notice, your child will not be dropped from the program. A signature is required to drop a child from the program; you must fill out a drop form yourself. ATS (AUTOMATIC TRANSFER SYSTEM) BANK AGREEMENT

1.The After School Conservatory Board of Managers may, at their discretion, adjust the monthly rate applicable to my category of service. I understand that I will receive at least four weeks notice prior to any such change.

2. Should any draft not be honored by my bank or credit card for any reason, I realize that I am still responsible for

that payment, plus a $25.00 service charge applied by the After School Conservatory. This is in addition to any service fee charged by my bank.

3. This card may be used for any unpaid After School Conservatory balances.

4. All enrollees- please attach a voided check from your bank to this form for bank draft.

5. Any changes must be made in writing to the After School Conservatory with a 15 day notice.

Registration Fee

A non-refundable, one-time registration fee of $30 is required when you register.

Payment Schedule

ATS draft will be made once a month on the 1st of the month. Cash/check payments are also due following the same schedule.

Absence allowance

You are allowed to freeze your account for 1 month without having to pay registration fees repeatedly when you return. We do not credit for time missed other than when it was notified in advance due to emergency reasons.

Delinquent payments

Late payment penalty of $25 will be assessed for payments received after the 5th of each month. Monthly fees must be paid in full on the 1st of each month.

Bad checks or Drafts

A $25 fee will be assessed for checks or ATS drafts rejected because of insufficient funds. This is in addition to any late payment penalties. Members are expected to pay immediately the amounts due resulting from rejected drafts or checks plus the assessment.

Make-ups

Students who are unable to attend a lesson or class due to an emergency such as an illness must notify the school at least 24 hours in advance in order to schedule a make-up lesson. We are only able to offer one alternative date for the make-up lesson, which cannot be negotiated. If a student is unable to attend this date for any reason, they forfeit the make-up lesson.

Refunds/Cancellations

Refund for cancellations will be made only when a member withdraws from the program and has a prepaid balance. You must give a written notice to the after school conservatory front desk 15 working days in advance. A full refund can not be given without a full 15 days notice.
